^{Ixilis was by FAR the hardest boss for us, i believe it took us 15+ tries, it was a pain in the ass, there has been no boss that hard for us. Second, comes The Thrall, but took 3 tries i believe, all other bosses were quite easy, in general too easy and 75% of them done at the first try.}

By upgrading one item from each of your equipment slots to the point where you need Hardened Iron I believe. Upgrade one item in each slot to the level that requires hardened iron. If by that point you're already at the end of the game, then re-roll your world (or re-roll some adventure worlds) and you should start getting regular hardened iron

That being said, I understand this game has some progression mechanics that warp difficulty levels. Is Nightmare possible on a first run? Or is Hard the limit for fresh ^{Enemies give 30% more XP on Nightmare. 50% more on Apocalypse. Hard is 10-15%, I don't remember. Lumenite drops are increased for each difficulty level.
